Making an appointment
Before making an appointment, review our healthcare provider partner profiltes - their locations, their hours, their prices, etc. Links to each provider's page and their prices are provided.
When you are ready to make an appointment:
- Call the phone number at the location where you’d like schedule an appointment.
- Inform the person taking your appointment that you are a self-pay patient, the type of service you need, and confirm the advertised price that you saw here.
- When visiting any Wal-Mart pharmacy or Wal-Mart vision center, remember to take your Wal-Mart Pharmacy Card. You must present that to the pharmacist or vision center attendant. You will receive your Wal-Mart Pharmacy Card by e-mail soon after purchasing a health insurance policy through us. It is a free, value-added service for all Certus Health clients.
- You must pay in cash or with credit card at time of service. There will be no billing. This is the key to how our healthcare provider partners are able to offer you such a good deal.
